What You Will Do
- Deliver accurate patient assessments and care plans in a remote setting
- Provide timely, empathetic patient communication and education
- Monitor patient progress and proactively identify risks
- Ensure complete, compliant, and accurate clinical documentation
- Escalate high-risk cases within defined SLAs
- Collaborate with cross-functional healthcare teams to develop and implement patient care plans
- Manage patient care plans and follow-ups independently
- Work within strict clinical and documentation standards
- Develop and maintain accurate and up-to-date patient records
- Participate in ongoing training and education to stay current with best practices in patient care
- Utilize Excel to track patient data and outcomes

- Active AANP certification
- Proven experience in patient care (telehealth experience preferred)
