What You Will Do
- Develop and implement patient-centered care plans that incorporate the latest medical research and digital health technologies.

- Facilitate communication among care teams, patients, and families through phone, email, and video conferencing.
- Conduct regular assessments to identify patient needs and adjust care plans accordingly.
- Collaborate with cross-functional teams to develop and implement new care pathways and digital solutions.
- Participate in quality improvement initiatives to enhance care delivery and patient satisfaction.
- Stay up-to-date with the latest advancements in digital health and care coordination to continuously improve practice.
- Provide education and support to patients and families on disease management, medication adherence, and healthy lifestyle practices.
- Work closely with the IT department to troubleshoot technical issues and optimize the use of digital health tools.
What We Are Looking For
- 2+ years of experience in care coordination, case management, or a related field.
- Strong understanding of healthcare systems, including Medicare, Medicaid, and private insurance.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse patient populations.
- Proficiency in digital health records (EHRs) and care coordination software.
- Ability to work independently and as part of a multidisciplinary team.
- Strong analytical and problem-solving skills, with the ability to interpret data and make informed decisions.
- Knowledge of healthcare regulations, including HIPAA and patient privacy laws.
- Bachelor's degree in a healthcare-related field, such as nursing, health administration, or public health.
- Certification in care coordination or a related field (e.g., CCM, RN) is preferred but not required.
Nice to Have
- Experience with telehealth technologies and remote patient monitoring systems.
- Familiarity with population health management principles and strategies.
- Certification in health coaching or a related field.
- Bilingual or multilingual skills, with the ability to communicate effectively with patients who speak languages other than English.
