About the Role

Day-to-day, you will work closely with healthcare providers, patients, and families to develop and implement care plans that meet the unique needs of each individual. Your ability to navigate complex healthcare systems, utilize digital health records, and facilitate seamless communication among care teams will be crucial in this position.

What You Will Do

• Develop and implement patient-centered care plans that incorporate the latest medical research and digital health technologies.
• Coordinate care services with healthcare providers, patients, and families to ensure seamless transitions and high-quality outcomes.
• Utilize digital health records and data analytics to inform care decisions and improve patient outcomes.
• Facilitate communication among care teams, patients, and families through phone, email, and video conferencing.
• Conduct regular assessments to identify patient needs and adjust care plans accordingly.
• Collaborate with cross-functional teams to develop and implement new care pathways and digital solutions.
• Participate in quality improvement initiatives to enhance care delivery and patient satisfaction.
• Stay up-to-date with the latest advancements in digital health and care coordination to continuously improve practice.
• Provide education and support to patients and families on disease management, medication adherence, and healthy lifestyle practices.
• Work closely with the IT department to troubleshoot technical issues and optimize the use of digital health tools.

What We Are Looking For

• 2+ years of experience in care coordination, case management, or a related field.
• Strong understanding of healthcare systems, including Medicare, Medicaid, and private insurance.
• Excellent communication and interpersonal skills, with the ability to work effectively with diverse patient populations.
• Proficiency in digital health records (EHRs) and care coordination software.
• Ability to work independently and as part of a multidisciplinary team.
• Strong analytical and problem-solving skills, with the ability to interpret data and make informed decisions.
• Knowledge of healthcare regulations, including HIPAA and patient privacy laws.
• Bachelor's degree in a healthcare-related field, such as nursing, health administration, or public health.
• Certification in care coordination or a related field (e.g., CCM, RN) is preferred but not required.

Nice to Have

• Experience with telehealth technologies and remote patient monitoring systems.
• Familiarity with population health management principles and strategies.
• Certification in health coaching or a related field.
• Bilingual or multilingual skills, with the ability to communicate effectively with patients who speak languages other than English.
