About the Role

As a Care Manager at Salvo Health, you will be at the heart of delivering high-quality patient care, working closely with a multidisciplinary team of registered dietitians, partner physicians, and other healthcare professionals. Your primary focus will be on supporting patients with chronic diseases, ensuring they receive the best possible care and support. What You Will Do

  • Develop and implement comprehensive care plans for patients with chronic diseases
  • Conduct regular assessments of patient needs and adjust care plans accordingly
  • Collaborate with registered dietitians and partner physicians to ensure coordinated care
  • Provide education and support to patients on disease management and healthy lifestyle practices
  • Utilize digital applications such as Slack, Coda, and Google Workspace to manage patient data and communications
  • Participate in continuous quality improvement initiatives to enhance patient outcomes
  • Analyze patient data to identify trends and areas for improvement in care delivery
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions
  • Stay updated on the latest developments in chronic disease management and incorporate this knowledge into practice
  • Work proactively to address barriers to care and ensure patient engagement

What We Are Looking For

  • 2+ years of experience as a Licensed Practical Nurse (LPN) or equivalent nursing license
  • 2+ years of experience in patient-facing or customer-facing roles
  • Compact state license required, with additional licensing potentially needed
  • Bilingual (Spanish-speaking) candidates are highly valued
  • Excellent customer relation skills, as well as written and verbal communication skills
  • Knowledge of medical terminology and proficiency in general medical office procedures
  • Familiarity with digital applications and strong analytical and problem-solving skills
  • Self-motivated, results-oriented, and a strategic thinker
  • Personal passion for health and wellness topics
  • Authorization to work in the United States

Nice to Have

  • Experience working in telehealth or a healthcare startup environment
  • Experience working with GI, weight management, and/or anti-obesity medications such as GLP-1s
  • Certification in chronic disease management or a related field
  • Experience with electronic health records (EHRs) and practice management systems
