About the Role
The Case Coordinator Family Supports role at VON Canada is a full-time, remote position that requires a dedicated and skilled professional to provide comprehensive assessments and support plans for clients. As a key member of the Family Supports team, the successful candidate will work closely with clients, caregivers, and other stakeholders to develop and implement personalized care plans that meet the unique needs of each individual. This role is critical in ensuring that clients receive the support and resources they need to achieve their goals and maintain their independence.The Case Coordinator will be responsible for conducting thorough assessments, including the use of standardized tools such as the interRAI CHA and supports Intensity Scale. They will also develop and maintain comprehensive care plans, ensuring that clients receive the right level of support and resources to meet their needs. This role requires a high level of communication and interpersonal skills, as well as the ability to work independently and as part of a team.
As a Case Coordinator, you will be working with a diverse range of clients, each with their unique needs and challenges. You will need to be flexible, adaptable, and able to think critically and creatively to develop innovative solutions that meet the complex needs of your clients. Your ability to work collaboratively with other healthcare professionals, community agencies, and service providers will be essential in ensuring that your clients receive the best possible support and care.
What You Will Do
- Conduct comprehensive assessments using standardized tools and conversation-based methodology
- Develop and maintain comprehensive care plans that meet the unique needs of each client
- Facilitate access to community resources and services, including referrals and navigation
- Collaborate with clients, caregivers, and other stakeholders to develop and implement personalized care plans
- Monitor client progress and adjust care plans as needed
- Collect and report relevant data to inform program outcomes and evaluate effectiveness
- Provide education and support to clients and caregivers on self-management and health promotion
- Develop and maintain effective relationships with community agencies and service providers
- Participate in quality improvement initiatives to enhance the delivery of support services
- Adhere to VON Canada's policies, procedures, and standards for community support services
- Maintain accurate and confidential records, including client files and documentation
What We Are Looking For
- 2-3 years of experience in a related field, such as social work, healthcare, or education
- Strong assessment and care planning skills, with experience in developing comprehensive support plans
- Excellent communication and interpersonal skills, with the ability to work with diverse clients and stakeholders
- Ability to work independently and as part of a team, with a high level of flexibility and adaptability
- Strong problem-solving and critical thinking skills, with the ability to think creatively and develop innovative solutions
- Experience working with community agencies and service providers, with a strong understanding of the healthcare system and social services
- Strong knowledge of client-centered care and self-management principles
- Ability to maintain accurate and confidential records, with a high level of attention to detail
- Familiarity with electronic client information systems and other technology platforms
Nice to Have
- Experience working with clients with developmental disabilities and complex needs
- Knowledge of the interRAI CHA and supports Intensity Scale, as well as other standardized assessment tools
- Certification in a related field, such as social work or healthcare
- Bilingual or multilingual skills, with the ability to communicate with clients and stakeholders in multiple languages
- Experience working in a remote or virtual environment, with a high level of self-motivation and discipline
