About the Role
The Case Manager position at Independent Health is a vital part of the healthcare team, responsible for applying the six essential activities of case management to ensure high-quality patient care. This role involves working closely with patients, healthcare providers, and other stakeholders to assess, plan, implement, coordinate, monitor, and evaluate patient care. As a Case Manager, you will be responsible for managing a caseload of patients, developing care plans, and coordinating services to meet the unique needs of each patient.The Case Manager will work in a fast-paced environment, collaborating with interdisciplinary teams to provide comprehensive care. This role requires strong clinical knowledge, excellent communication skills, and the ability to work independently. What You Will Do
- Assess patients' social and medical history to identify their immediate and long-term needs
- Develop plans of care based on clinical assessments, considering physical and psychosocial needs, benefit plans, and cost-benefit analysis factors
- Continuously evaluate and update plans of care to reflect changing patient needs
- Coordinate with healthcare providers, patients, and families to ensure effective care delivery
- Monitor the quality of care and effectiveness of care being provided
- Ensure compliance with regulatory and privacy standards
- Establish professional working relationships with interdisciplinary team members
- Communicate case objectives to involved parties
- Act as a patient advocate, understanding and identifying potential complications
- Apply principles of the American with Disabilities Act and federal legislation affecting individuals with disabilities
- Establish available support systems and research community resources to assist patients in achieving maximum recovery
What We Are Looking For
- Registered Nurse (RN) with active, current, unrestricted license in a Nurse Licensure Compact (NLC) state
- Bachelor's degree preferred
- Case Management Certification (CCM) required, or willingness to obtain within three years of commencing employment
- Three years of clinical RN experience in a med/surg or ambulatory care setting
- Clinical experience in Behavior Health, Transplant, Neo-Natal, or Oncology preferred
- Previous experience in managed care a plus
- Excellent problem-solving abilities, coupled with solid time management skills
- Excellent written, verbal, and interpersonal communication skills
- Ability to work independently and proficiently, with flexibility in work schedules and assignments

- Experience with electronic health records (EHRs) and care management software
- Knowledge of assistive and adaptive equipment
- Familiarity with community resources and support services
- Experience working in a remote or virtual care setting
