About the Role

As a Clinical Care Navigator at Lyra Health, you'll be responsible for managing client crises, providing in-the-moment phone support to individuals with complex behavioral health issues. Your expertise will ensure client safety and connection to high-quality, evidence-based providers and facilities. What You Will Do

  • Manage client crises through evidence-based interventions, providing brief risk assessment, safety planning, and de-escalation techniques.
  • Identify roadblocks to clinical care options for clients, using Lyra's proprietary software to connect them to the ideal treatment based on their specific needs and preferences.
  • Independently manage a caseload of complex, multi-need clients, ensuring timely and effective support.
  • Develop and implement individualized plans to address client needs, collaborating with other healthcare professionals as necessary.
  • Provide education and resources to clients, empowering them to take an active role in their mental health care.
  • Maintain accurate and up-to-date records of client interactions, ensuring seamless continuity of care.
  • Participate in regular team meetings and training sessions to stay updated on best practices and new developments in mental health care.
  • Collaborate with other departments to ensure a comprehensive approach to client care.

What We Are Looking For

  • Master's degree in clinical social work or equivalent, with LCSW or equivalent independent clinical licensure (e.g., LMFT, LPC, LPCC, etc.).
  • Bachelor's degree in nursing (RN) with significant experience in psychiatric care.
  • 2+ years of experience in psychiatric intake, assessment, and related fields, with a strong background in crisis management.
  • 2+ years of post-licensure experience, preferably in an EAP or clinical call center setting.
  • Significant experience managing high-risk clinical situations, including suicidality and mandated reporting.
  • Excellent written and verbal communication skills, with the ability to work effectively in a remote environment.
  • Proficiency in Excel and other software applications, with a willingness to adopt new technology platforms.
  • Bilingual, Spanish-speaking candidates are strongly preferred but not required.

Nice to Have

  • Experience working with electronic health records (EHRs) and other healthcare software applications.
  • Knowledge of mental health care systems and regulations, including HIPAA compliance.
  • Certification in crisis management or a related field, such as CPR or FIRST AID.
