About the Role
As a Clinical Pharmacist at Centene, you will play a pivotal role in managing populations of patients who meet high-risk criteria, aiming to improve disease state outcomes and enhance medication-related safety. Your expertise will be crucial in ensuring safe, effective, and cost-effective medication therapy for all patients under your care. This position requires a deep understanding of clinical trends, the ability to educate clinical teams and prescribers, and the capacity to develop educational materials for intervention programs.The role is part of the Medical Management/Health Services team, which is dedicated to providing comprehensive care to Centene's members. Your day-to-day activities will include reviewing patient drug profiles, collaborating with physicians and other prescribers, and conducting research to identify opportunities for cost management and process improvement.
What You Will Do
- Manage and coordinate patient care activities to achieve optimal patient outcomes and minimize hospitalizations.
- Conduct thorough reviews of patient medication regimens to identify potential gaps in care, high-risk medications, and areas for improvement in adherence and compliance.
- Collaborate with healthcare providers to implement safe and effective medication therapies, considering formulary information and patient benefits.
- Develop and implement strategies to improve medication adherence and health outcomes, utilizing data and clinical insights.
- Participate in the development of educational materials and programs aimed at enhancing patient care and medication management.
- Engage in outreach activities to clarify medication regimens and provide recommendations to healthcare providers.
- Utilize clinical expertise to provide recommendations on gaps in care, high-risk medications, and therapeutic substitutions.
- Stay updated on current and emerging clinical trends and guidelines to ensure best practices in patient care.
- Analyze data to identify trends and areas for improvement in patient outcomes and medication management.
- Work closely with the quality department to improve HEDIS metrics related to medication adherence and routine health visits.
What We Are Looking For
- A Bachelor’s degree or advanced degree (Pharm.D., M.S.) in Pharmacy.
- At least 2 years of experience in mail order, retail, hospital, or managed care pharmacy, or completion of a pharmacy residency program.
- Strong knowledge of pharmacotherapy, pharmacokinetics, and pharmacodynamics.
- Excellent communication and interpersonal skills, with the ability to collaborate effectively with healthcare teams.
- Proficiency in utilizing clinical information systems and electronic health records.
- Ability to work in a fast-paced environment and prioritize tasks effectively.
- Understanding of healthcare regulations and standards, including HIPAA.
- Experience with medication therapy management and disease state management programs.
