About the Role
As a Clinical Pharmacist at Sentara Health, you will be responsible for delivering high-quality patient care through comprehensive medication management. You will work closely with the healthcare team to identify and address medication-related issues, ensuring optimal patient outcomes. What You Will Do
- Provide comprehensive medication management services to patients, including counseling and education on medication use and side effects
- Collaborate with healthcare providers to develop and implement individualized treatment plans, taking into account patient-specific factors and medication therapy
- Conduct thorough medication reviews to identify potential issues and opportunities for improvement
- Work with patients to address medication-related concerns and questions, providing personalized support and guidance
- Develop and maintain accurate patient records, documenting medication therapy and patient outcomes
- Participate in quality improvement initiatives, identifying areas for improvement and implementing changes to optimize patient care
- Stay up-to-date with the latest developments in pharmacotherapeutics, attending conferences and completing continuing education courses as needed
- Collaborate with other healthcare professionals to develop and implement disease management programs, focusing on prevention and early intervention
- Analyze data and trends to identify opportunities for improvement in patient care, developing and implementing strategies to address these gaps
What We Are Looking For
- Bachelor's degree in Pharmacy (PharmD) from an accredited institution
- Current licensure as a pharmacist in the United States, with the ability to obtain Virginia licensure within 90 days of start date
- Strong foundation in pharmacotherapeutic principles, medical terminology, and population health management
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, healthcare providers, and other stakeholders
- Experience with online pharmacy adjudication systems and electronic health records
- Strong analytical and problem-solving skills, with the ability to think critically and make informed decisions
- Ability to work independently and as part of a team, with a strong commitment to collaboration and teamwork
- Experience working with Medicare STARS and/or other healthcare quality initiatives
Nice to Have
- Advanced certification in a relevant area, such as ambulatory care or infectious disease
- Experience working in a remote or telehealth setting, with a strong understanding of the unique challenges and opportunities presented by these models
- Familiarity with quality improvement methodologies and performance measurement tools
- Experience working with diverse patient populations, including those with complex medical and social needs
