About the Role
As a Clinical Pharmacist at Shields Health Solutions, you will play a vital role in providing integrated support to patients and physicians, focusing on improving the quality of patient care through personalized relationships and evidence-based treatments. Your expertise will be crucial in developing and refining disease and drug-specific clinical protocols, maximizing patient adherence, and promoting cost-effective care. You will work closely with a team of dedicated professionals, including clinicians, prescribers, and support staff, to ensure seamless care coordination and optimal patient outcomes.The role of a Clinical Pharmacist is multifaceted, requiring a deep understanding of pharmacology, disease management, and patient-centered care. You will be responsible for reviewing and interpreting patient care notes, developing care plan recommendations, and monitoring patient adherence across various disease categories. Your expertise in Excel will be essential in analyzing data, identifying trends, and making informed decisions to improve patient care.
What You Will Do
- Review and interpret clinician's patient care notes to identify supportive clinical data for prior authorization approvals and care plan development
- Develop and refine disease and drug-specific clinical protocols to maximize patient adherence and promote evidence-based care
- Monitor ongoing patient adherence across all sites and disease categories, focusing on oncology and other specialty-specific disease states
- Evaluate electronic medical records to review drug and disease interactions, medication selection, labs/vitals, allergies, and RX orders/renewals
- Recommend alternative therapeutic drug regimens and disease-specific treatments according to guidelines
- Conduct medication therapy management, including reviewing and monitoring medication regimens, advising clinicians on evidence-based therapy, and documenting interventions
- Provide clinical interventions, such as comprehensive pharmacist consultations, motivational interviewing/coaching, and medication reconciliation
- Oversee daily activities of support personnel, ensuring accuracy and compliance with protocols
- Identify and resolve insurance-related issues, recognizing situations where financial assistance can be applied
- Develop and maintain current knowledge by participating in education programs and reading professional publications
What We Are Looking For
- Valid North Carolina pharmacist license
- Strong foundation in pharmacology, disease management, and patient-centered care
- Excellent communication and interpersonal skills
- Proficiency in Excel and electronic medical records
