About the Role
The Clinical Pharmacist position at Lehigh Valley Health Network is a unique opportunity to join a renowned healthcare organization that is dedicated to delivering exceptional patient care. What You Will Do
- Coordinate pharmacy initiatives involving registry or high-risk patients identified within the LVPHO practices
- Develop and implement pharmacy initiatives directed at promoting high-value, high-quality, safe, and cost-efficient medication therapy management
- Apply subject matter expertise in clinical pharmacy, therapeutic alternatives, utilization management, medication adherence strategies, and Medication Therapy Management
- Critically review drug utilization data provided by insurers and analytic teams
- Evaluate patient EMR profiles for appropriateness of medication therapy and conduct patient appointments
- Provide and document patient drug therapy recommendations
- Identify prescribing patterns that may lead to improper drug selection and implement strategies to resolve cost barriers
- Interact with patients, providers, and pharmacists to positively impact CMS STARS, PQA, or HEDIS quality measures
- Work with the Population Health Director and ICC Pharmacy Manager to implement strategic population health pharmacy plans
- Provide education to providers and staff via presentations, newsletters, formal utilization reviews, and one-on-one interactions
What We Are Looking For
- Bachelor's Degree in Pharmacy or Doctorate of Pharmacy
- 2 years of relevant experience in patient care or population health, or 1 year of PGY-1 Residency experience
- Skilled in critical appraisal of medical literature and capable of independently reaching therapeutic decisions
- Ability to accurately monitor medications and make independent judgments under stressful situations
- Knowledge of laws, regulations, and accreditation standards related to medication management
- Registered Pharmacist in the state of Pennsylvania
- Excellent communication and interpersonal skills
Nice to Have
- 5 years of relevant experience in patient care or population health
- Prior experience working with health insurance payer contracts
- Completion of a PGY-1 Residency or formal training in population health
- Experience with data analysis and performance improvement projects
