Job Description
About the Role
The Clinical Pharmacist plays a critical role in providing integrated support to patients and physicians, with a focus on improving medication adherence and promoting evidence-based care. This is achieved through the development of personalized relationships with patients and prescribers, as well as the creation of tailored care plans. As a key member of the healthcare team, the Clinical Pharmacist will work closely with other healthcare professionals to ensure seamless patient care.The day-to-day responsibilities of this role involve reviewing and interpreting patient care notes, developing and refining disease-specific clinical protocols, and monitoring patient adherence to medication regimens. The Clinical Pharmacist will also be responsible for evaluating electronic medical records, identifying potential drug interactions, and recommending alternative therapeutic regimens as needed.

What You Will Do
- Review and interpret clinician's patient care notes to identify supportive clinical data for prior authorization approvals and care plan recommendations
- Develop and refine disease and drug-specific clinical protocols to maximize patient adherence and promote evidence-based care
- Monitor patient adherence to medication regimens across all sites and disease categories, with a focus on oncology and other specialty-specific disease states
- Evaluate electronic medical records to review drug and disease interactions, medication selection, labs/vitals, allergies, and other relevant information
- Recommend alternative therapeutic regimens and disease-specific treatments according to guidelines
- Monitor and evaluate therapies for side effects or other medication-related issues
- Provide medication therapy management, including reviewing and monitoring medication regimens and advising clinicians on evidence-based therapy
- Develop and implement clinical interventions, such as comprehensive pharmacist consultations and motivational interviewing/coaching
- Oversee the daily activities of support personnel and verify accuracy of delivery coordination slips, prior authorizations, and ongoing PA log activity
- Identify and resolve insurance-related issues, recognizing situations where financial assistance can be applied
What We Are Looking For
- Strong analytical and problem-solving skills, with the ability to exercise good judgment and possess excellent verbal and written communication skills
- Deep knowledge of specialty medication pharmacology, local/state/federal pharmacy regulations, and experience with electronic medical records
- Ability to manage processes, exhibit strong technical and clinical skills, and excellent leadership and management abilities
- Experience with medication therapy management, clinical protocol development, and disease management
- Strong understanding of HIPAA rules and regulations, with the ability to maintain confidentiality and comply with local, state, and federal drug laws
- Ability to work in a fast-paced environment, prioritize tasks, and manage multiple projects simultaneously
