About the Role

The Community Health Partner role at Cityblock Health is a unique opportunity to support integrated social care for members with complex health needs. As a Community Health Partner, you will work collaboratively with RN Care Managers to provide comprehensive support to members, addressing social determinants of health and developing care plans to achieve optimal health outcomes. You will work remotely, utilizing digital platforms to document interactions, track tasks, and communicate effectively with the care team and members.

What You Will Do

  • Collaborate with RN Care Managers to support members with complex medical and behavioral health needs
  • Manage a caseload of members, identifying and addressing social, behavioral, and environmental barriers to optimal health
  • Develop and implement care plans in partnership with members, addressing social needs and supporting members in achieving their care goals
  • Conduct self-efficacy and condition-specific assessments, including behavioral health screenings
  • Refer members to appropriate behavioral health programs as needed
  • Support members in closing quality gaps by assisting with provider appointments, diagnostic or lab testing, and referrals to community resources
  • Meet members in community settings as needed, serving as an extension of the care team
  • Utilize care facilitation and scheduling platforms to document interactions, track tasks, and communicate effectively
  • Identify and escalate members needing additional support or higher levels of care

What We Are Looking For

  • 2+ years of progressive experience in care coordination, community health, health promotion, or a related field
  • Strong critical thinking and problem-solving skills
  • High-quality written, verbal, and presentation communication skills
  • Previous experience with documentation and digital platforms
  • Ability to work autonomously and as part of a remote team
  • Strong member advocacy and compassion skills
  • Experience working with diverse populations and communities

Nice to Have

  • Experience with care facilitation and scheduling platforms
  • Knowledge of behavioral health screenings and assessments
  • Certification in a related field, such as care coordination or health education
