Job Description

About the Role

The Community Health Worker, Housing Specialist, is a vital part of Humana's care management team. This role involves working closely with members to address their housing concerns, connecting them with community resources, and providing guidance on navigating healthcare and social service systems. As a key member of the care team, you will apply a hands-on approach to member engagement, supporting case management functions and addressing social determinants of health needs. You will report to a Manager of Care Management and work collaboratively with other associates to ensure effective program administration.

The role entails traveling up to 25% to member homes and facilities, conducting in-person assessments, and evaluating the home environment to understand member care needs and preferences. You will also build and maintain relationships with providers and community resources to support member referrals and identify resource gaps.

What You Will Do

  • Conduct in-person assessments to understand member care needs, preferences, socioeconomic barriers, and evaluate the home environment
  • Assist members in navigating healthcare and social service systems, coordinating access to basic needs with a focus on housing
  • Identify and address barriers to housing, ensuring members can attend their appointments
  • Provide social support to boost members' morale and sense of self-worth, serving as a trustworthy, reliable, non-judgmental, consistent, and accepting team member
  • Support member independence through appropriate education and coaching
  • Conduct research and in-person outreach to locate difficult-to-contact members to increase assessment completion and participation in clinical programs
  • Build and maintain relationships with providers and community resources to support member referrals and implement community assessments to identify resource gaps
  • Regularly travel to conduct member visits and community-based visits as needed to ensure effective program administration
  • Work collaboratively with other associates as a member of the care support team
  • Leverage your knowledge of the community and shared life experiences to close gaps in care
  • Use your skills to make an impact, applying a hands-on approach to member engagement
  • Address social determinants of health needs, improve self-management of chronic conditions, and promote prevention and health education tailored to the needs of the communities served

What We Are Looking For

  • Minimum two years of experience with community resources, health agencies, or social services
  • Proficiency in Microsoft Office, including Teams, Excel, PowerPoint, Outlook, and Word
  • Ability to build rapport with members and community partners
  • Respect for cultural and demographic diversity
  • Strong written and verbal communication skills and advocacy for members at all care levels
  • Must reside in the United States
  • Ability to travel up to 25%
  • Separate room with a locked door that can be used as a home office to ensure continuous privacy while working
  • Community Health Worker training and/or certificate or willingness to complete within one year

Nice to Have

  • Bachelor's degree in social work or a related field
  • Familiarity with state Medicaid program guidelines
  • Experience engaging with Medicaid enrollees, including those with physical health and behavioral health needs and varied health literacy
