About the Role
The Coordinator Health Care role at Highmark Health is a critical position that involves providing care planning, coordination, and follow-up for patients at risk for poor outcomes or experiencing poor coordination of services. As a key member of the Practice Team, you will work closely with hospital, long-term care, specialty services, home health services, patient family, cultural, and community resources to ensure comprehensive care for managed patients. Your expertise will be essential in maintaining accurate documentation, promoting Practice Team awareness, and ensuring patient safety and follow-through on care plans.The health care industry is rapidly shifting towards more personalized and coordinated care, and this role is at the forefront of that movement. What You Will Do
- Complete structured assessments of medical, bio-psychosocial support, and self-management support needs for patients.
- Coordinate with and act as an advocate/liaison/contact to hospital, long-term care, specialty services, home health services, patient family, cultural, and community resources for care-managed patients.
- Accurately maintain appropriate documentation on care coordination to promote Practice Team awareness and ensure patient safety and follow-through on care plans.
- Provide registration and billing functions, assisting patients in problem-solving potential issues related to the health care system, financial, and psychological barriers.
- Collaborate with primary care to establish and update a shared care plan, providing support for improving health behaviors and self-management skills.
- Participate in goal setting, action planning, and problem-solving with patients and the Practice Team.
- Provide clinical workflow oversight and support to ensure seamless care delivery.
- Perform other duties as assigned or requested by the Practice Team.
What We Are Looking For
- Graduate of a Medical Assistant or LPN Program, or equivalent experience.
- At least 5 years of experience in a Medical Assistant or related role.
- Strong communication and problem-solving skills, with the ability to empathize with patients and families.
- Ability to work independently in a remote setting and collaborate effectively with cross-functional teams.
- Proficiency in electronic health records and other health care software systems.
- Knowledge of health care regulations, including HIPAA, and adherence to confidentiality and privacy standards.
Nice to Have
- Experience working with diverse patient populations and cultural backgrounds.
- Certification in case management or a related field.
