As a Crisis Care Manager, you will be part of a dedicated team that provides emotional support, care coordination, and connection to resources for families of children with behavioral and medical complexities. Your role will be crucial in helping these families navigate the often-complex healthcare system, ensuring they receive the best possible care. The Crisis Care team is a centralized unit that consists of therapists, case managers, and other healthcare professionals. You will work closely with this team to provide an extra layer of support to families, acting as a bridge between them and the healthcare system. Your work will be remote, with a schedule that includes shift work, allowing you to provide 24/7 support to those who need it. The role of a Crisis Care Manager is multifaceted and requires a deep understanding of the healthcare system, as well as the ability to work with diverse families and healthcare providers. - Provide emotional support and care coordination to families of children with behavioral and medical complexities

- Communicate effectively with families, healthcare providers, and other stakeholders to ensure seamless care coordination

- Stay up-to-date with the latest developments in pediatric care and incorporate this knowledge into your practice

- Collaborate with other healthcare professionals to develop and implement best practices in crisis care management
- Utilize digital skills to streamline care coordination and communication with families and healthcare providers
What We Are Looking For
- Experience working in pediatric care, preferably in a role that involves care coordination or case management
- Strong understanding of the healthcare system and the challenges faced by families of children with special healthcare needs
- Ability to work remotely and manage a diverse caseload
