About the Role

The Dementia Care Nurse Practitioner role at Synapticure is a unique opportunity to make a meaningful impact on the lives of patients and caregivers affected by neurodegenerative diseases. As a key member of the interdisciplinary care team, you will work closely with neurologists, dementia specialists, and other healthcare professionals to provide comprehensive and compassionate care. In this role, you will be responsible for diagnosing, treating, and managing dementia and other acute episodic illnesses through comprehensive and acute care visits. You will also facilitate appointments with consultants, provide dementia education and management skills to caregivers, and take urgent calls during daytime hours. What You Will Do

  • Diagnose, treat, and manage dementia and other acute episodic illnesses through comprehensive and acute care visits
  • Provide telehealth comprehensive and acute visits with patients and caregivers
  • Facilitate appointments with consultants when treatment plans need to be reassessed
  • Offer dementia education and management skills to caregivers
  • Take urgent calls during daytime hours and manage or triage problems
  • Communicate with community primary care physicians and other healthcare professionals
  • Order and interpret lab and diagnostic testing, adjusting treatment plans accordingly
  • Collaborate with Synapticure's neurologists, dementia specialists, and interdisciplinary care team to deliver exceptional patient care
  • Maintain accurate and compliant documentation within the electronic medical record (EMR) system
  • Participate in weekly debriefing sessions with the clinical team to review cases, discuss issues, and share best practices

What We Are Looking For

  • Master's of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
  • Current Nurse Practitioner credentialing body certificate
  • Active, unrestricted APRN licensure
  • 2 years of experience as an APRN in primary care, internal medicine, geriatric care, neurology, or equivalent
  • Working knowledge of Geriatrics and care of Geriatric/Neurology patients
  • Ability to take medical histories, perform examinations, assessments, and implement diagnostic and therapeutic plans
  • Excellent verbal and written communication, organizational, and interpersonal skills
  • Ability to work effectively in a fast-paced, team-based environment, prioritizing tasks and completing them in a timely manner

Nice to Have

  • Experience as an APRN in a gerontology setting
  • Knowledge of electronic medical records and innovative technologies
  • Certification in gerontological nursing or a related field
  • Experience with care coordination and behavioral health support
