About the Role

The General Practitioner Telehealth Case Conferencing Clinic role at Proactive Health is a unique opportunity to work with patients who have complex, multi-system health needs. As a General Practitioner, you will provide clinical leadership across in-clinic consultations and structured telehealth case conferencing, working alongside allied health professionals and specialists to deliver coordinated, evidence-based care plans.

The role is structured around two days per week - an in-clinic day and a remote telehealth case-conferencing day - with some flexibility to accommodate your availability. You will work with veterans who often present with chronic pain, mental health comorbidities, musculoskeletal injuries, and the long-term effects of military service.

What You Will Do

  • Conduct comprehensive GP consultations for acute, chronic, and complex conditions within a Chronic Care Team framework
  • Deliver preventative health assessments, including cardiovascular risk, metabolic health, mental-health screening, and lifestyle risk modification
  • Undertake chronic disease management reviews and care planning, including GP Management Plans and Team Care Arrangements
  • Lead and participate in scheduled telehealth case conferences as part of multidisciplinary Chronic Care Teams
  • Initiate, review, and manage pathology and imaging investigations
  • Provide prescribing and medication management
  • Refer to allied health, specialists, and community services
  • Ensure clinical safety, prioritization, and escalation pathways are clearly identified and actioned

What We Are Looking For

  • Medical degree (MBBS or equivalent) and current registration with the Medical Board
  • Experience working with chronic disease management and telehealth
  • Ability to work independently and as part of a multidisciplinary team
  • Excellent communication and interpersonal skills
  • Experience with Medicare and DVA billing
  • Ability to work remotely and prioritize patient care
  • Strong organizational and time management skills
  • Ability to use Excel and other software applications

Nice to Have

  • Experience working with veterans and patients with complex health needs
  • Knowledge of the Coordinated Veterans' Care (CVC) Program
  • Experience with veteran-specific health assessments
  • Certification in chronic disease management
