About the Role
The Health Coach position at Cardiovascular Associates of America is a non-clinical role that supports patients with heart failure through education, behavior change, and engagement. As a key member of the care team, you will work closely with clinical team members to empower patients to better manage their condition and improve their overall well-being. What You Will Do
- Provide education and coaching related to heart failure, lifestyle modification, medication adherence, nutrition, physical activity, and self-management
- Establish short- and long-term goals with patients using shared decision-making and motivational interviewing techniques
- Encourage sustained patient engagement, self-advocacy, and behavior change throughout program participation
- Partner with the clinical care team to support patient care plans and escalate concerns appropriately
- Provide timely updates and documentation related to patient engagement and progress
- Participate in interdisciplinary team meetings
- Respond to patient communications in a timely, professional, and compassionate manner
- Build and maintain strong, trust-based relationships with patients and caregivers
- Consistently deliver a high-quality patient experience aligned with program standards
What We Are Looking For
- Bachelor's degree in health sciences or related field preferred
- NBC-HWC (National Board for Health & Wellness Coaching) certification required
- Experience in a healthcare, chronic disease management, or patient engagement setting preferred
- Cardiology or heart failure experience a plus
- Telehealth or remote patient engagement experience preferred
- Strong communication and interpersonal skills
- Ability to motivate and support behavior change in diverse patient populations
- Organized, adaptable, and comfortable managing multiple priorities
- Proficiency with electronic health records and standard computer applications
- Bilingual English/Spanish proficiency preferred
Nice to Have
- Experience working with patients with chronic diseases
- Knowledge of value-based care models and population health management
- Certification in a related field, such as health education or counseling
