The Hybrid Registered Nurse Clinical Care Manager will play a vital role in managing clinical care and coordinating services for patients.Responsibilities

- Coordinate services with providers, community partners, and member/caregivers
- Serve as a support resource for member and provider requests, escalations, or concerns
- Collaborate with the care team, providers, and partners to support member service requests
