About the Role
As a Licensed Practical Nurse (LPN) at Elaya Health, you will play a vital role in delivering high-quality patient care through Principal Care Management (PCM) services. Your day-to-day responsibilities will include reviewing patient charts, coordinating care with providers and families, and documenting patient interactions. What You Will Do
- Review patient charts, medications, labs, and care plans to identify areas for improvement
- Coordinate care with providers, nursing staff, pharmacies, and families to ensure seamless patient care
- Document patient interactions accurately and thoroughly, adhering to CMS guidelines
- Identify changes in condition and escalate concerns appropriately to ensure timely interventions
- Assist with chronic disease management and patient education to empower patients and their families
- Ensure PCM services are completed in compliance with CMS guidelines to maintain high standards of care
- Collaborate with the interdisciplinary team to develop and implement care plans tailored to each patient's needs
- Provide emotional support and counseling to patients and their families as needed
- Stay up-to-date with the latest developments in chronic care management and PCM to continuously improve patient outcomes
What We Are Looking For
- Active LPN license
- Nursing home or long-term care experience preferred
- Experience with chronic care management or PCM is a plus
- Strong communication and organizational skills
- Comfortable working independently in a remote environment
- EMR experience preferred
- Ability to work in a fast-paced environment and adapt to changing priorities
- Strong analytical and problem-solving skills
- Compassionate and empathetic approach to patient care
