About the Role

As a Mom and Baby Care Manager - RN at CareSource, you will play a vital role in collaborating with interdisciplinary care teams, providers, and community organizations to deliver high-quality, culturally competent care to members. Your primary focus will be on managing care plans, coordinating services, and facilitating communication between members, their natural support systems, and healthcare providers. You will work closely with the care team to assess member needs, develop personalized care plans, and implement effective interventions to improve health outcomes.

The Mom and Baby Care Manager role is critical in addressing the complex needs of members, particularly those with high-risk pregnancies, newborns, or other specialized care requirements. Your expertise as a registered nurse will be essential in navigating the healthcare system, identifying barriers to care, and developing creative solutions to address these challenges.

You will be part of a dynamic team that prioritizes person-centered care, health equity, and trauma-informed approaches. Your role will involve regular communication with members, caregivers, and healthcare providers via telephonic or electronic means. You will also participate in interdisciplinary care team meetings to ensure comprehensive care planning and coordination.

What You Will Do

  • Engage members and their natural support systems through strength-based assessments and trauma-informed care approaches
  • Develop and regularly update person-centered, individualized care plans in collaboration with the care team
  • Identify and manage barriers to care plan goals, implementing effective interventions based on clinical standards and best practices
  • Facilitate coordination, communication, and collaboration with members, care teams, and healthcare providers to achieve care plan goals
  • Educate members and their natural support systems about treatment options, community resources, insurance benefits, and other relevant information
  • Employ ongoing assessment and documentation to evaluate member response to care plans and progress
  • Monitor and promote effective utilization of healthcare resources through clinical variance and benefits management
  • Verify eligibility, enrollment history, demographics, and current health status of members
  • Complete psychosocial and behavioral assessments by gathering information from members, families, providers, and other stakeholders
  • Oversee timely psychosocial and behavioral assessments, care planning, and execution of member needs
  • Participate in meetings with providers to inform them of Care Management services and benefits available to members
  • Assist with ICDS model of care orientation and training for facility and community providers
  • Identify and address gaps in care and access, collaborating with healthcare professionals and providers to plan post-discharge care needs or facilitate transitions to appropriate levels of care

What We Are Looking For

  • Current RN licensure in the state of Nevada
  • 2+ years of experience in care management, preferably in a managed care or healthcare setting
  • Strong understanding of healthcare systems, including Medicaid and Medicare
  • Excellent communication, interpersonal, and problem-solving skills
  • Ability to work independently and as part of an interdisciplinary team
  • Proficiency in Microsoft Office, particularly Excel
  • Experience with programmatic interventions and care planning
  • Knowledge of cultural competence, health equity, and trauma-informed care principles
  • Strong analytical and critical thinking skills
