## About the Role Imagine Pediatrics is a tech-enabled, pediatrician-led medical group that is revolutionizing the way care is delivered to children with special health needs. As a Patient Navigator, you will play a critical role in ensuring that these children receive the highest level of care possible. Your day-to-day responsibilities will include providing clinical and clerical support to the care team, coordinating care with families and healthcare providers, and facilitating communication between all parties involved. The Patient Navigator role is a unique opportunity to make a meaningful difference in the lives of children and their families. You will be working closely with a team of dedicated professionals who share your passion for delivering exceptional care. With a focus on virtual first and in-home care, you will have the flexibility to work remotely while still being part of a dynamic team. The primary location of this position is remote, with an expected schedule of 3x12s (7:00am - 7:00pm CST) Monday-Wednesday or Wednesday-Friday, and quarterly travel to Houston, TX.
## What You Will Do - Provide clinical and clerical support to the care team, ensuring the delivery of high-quality, compassionate care - Coordinate care with families, healthcare providers, and other stakeholders to ensure seamless communication and collaboration - Facilitate communication between all parties involved in the care of children with special health needs - Develop and maintain relationships with families, healthcare providers, and other stakeholders to ensure exceptional care - Identify barriers to care and work to break them down, ensuring that children receive the care they need - Collaborate with the care team to develop and implement care plans that meet the unique needs of each child - Provide education and support to families and healthcare providers on the care of children with special health needs - Maintain accurate and up-to-date records of patient interactions and care plans - Participate in quarterly travel to Houston, TX for team meetings and training
## What We Are Looking For - 2+ years of experience in a healthcare-related field, preferably in pediatrics or a related field - Strong digital skills, including proficiency in electronic medical records and other healthcare software - Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders - Compassionate and empathetic, with a strong commitment to delivering exceptional care - Ability to work independently and as part of a team, with a strong focus on collaboration and communication - Strong problem-solving and critical thinking skills, with the ability to identify and address barriers to care - Familiarity with value-based care principles and practices - Experience working in a remote or virtual care environment
## Nice to Have - Experience working with children with special health needs or medical complexity - Certification in a related field, such as nursing or healthcare administration - Bilingual or multilingual, with the ability to communicate effectively with diverse patient populations - Experience with care coordination or case management
