The Chart Reviewer role is integral to the success of Daymark Health's mission to empower patients with dedicated care navigation, symptom-focused support, and social resources. As part of a dynamic team, you will work closely with other healthcare professionals to ensure that patients receive the most appropriate care.

What You Will Do

• Conduct retrospective and concurrent reviews of inpatient, outpatient, and ancillary medical records to assess eligibility for Daymark Health's programs.
• Apply established clinical criteria and program-specific guidelines to evaluate medical necessity and eligibility determinations.
• Accurately abstract and document relevant clinical findings, diagnoses, procedures, and treatment history from the medical record.
• Identify documentation gaps or inconsistencies and escalate cases requiring physician or supervisor review as appropriate.
• Maintain strict confidentiality of all patient health information in accordance with HIPAA regulations and Daymark Health's policies.
• Participate in audits, quality reviews, and process improvement initiatives as requested.
• Stay current on program eligibility criteria and relevant regulatory updates.
• Collaborate with the care team to ensure comprehensive care planning and coordination.
• Utilize electronic health record (EHR) systems and other digital tools to manage and review patient records efficiently.

What We Are Looking For

• An active, unrestricted Registered Nurse (RN) license in good standing.
• A minimum of 5 years of Oncology nursing experience, with experience in case management strongly preferred.
• Working knowledge of medical terminology and clinical documentation standards.
• Proficiency with electronic health record (EHR) systems.
• Comfort with tools such as Google Suite, Zoom, and Slack.
• Strong critical thinking and attention to detail, with the ability to review and interpret complex clinical documentation independently.
• A deep commitment to Daymark's mission to support cancer patients with dignity and compassion.
• Ability to manage multiple reviews simultaneously and meet established turnaround times.
• Meticulous documentation and organizational skills.
• Adaptability to work independently in a dynamic environment with minimal supervision.

• Experience with quality assurance and process improvement in a healthcare setting.
• Familiarity with value-based care models and their implementation.
• Certification in a relevant field, such as oncology nursing or case management.
• Experience with data analysis and reporting.
