About the Role
The Pharmacy Liaison plays a critical role in providing integrated off-site support to patients, pharmacists, and providers. This position requires a unique blend of clinical expertise, digital skills, and exceptional patient care. As a key member of the clinic team, the Pharmacy Liaison will work closely with patients to deliver personalized medication management services, utilizing various tools and applications to ensure seamless care.The role is centered around providing 'legendary' care to hospital patients, which entails a full suite of custom medication management services. This includes handling prior authorizations, financial assistance, patient medication on-boarding, and patient refill/renewal support. The Pharmacy Liaison will also educate prospective pharmacy patients on the benefits of using the hospital pharmacy, with the ultimate goal of growing the specialty pharmacies' patient population.
What You Will Do
- Provide exceptional patient care through personalized medication management services
- Utilize digital tools and applications to deliver seamless care
- Handle prior authorizations, financial assistance, and patient medication on-boarding
- Educate prospective pharmacy patients on the benefits of using the hospital pharmacy
- Grow the specialty pharmacies' patient population through effective patient care and education
- Collaborate with the clinic team to deliver comprehensive care
- Document patient information within the hospital EMR and other relevant systems
- Resolve insurance-related issues and complete benefit investigations
- Review patient needs for financial assistance and secure support when needed
- Stay up-to-date with the latest developments in digital health and patient care
What We Are Looking For
- Remote work experience and digital skills
- Clinical expertise in pharmacy or a related field
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong problem-solving and analytical skills
- Experience with electronic medical records (EMRs) and other digital health systems
- Knowledge of medication management and patient care principles
- Certification in pharmacy or a related field (preferred)
- 2+ years of experience in a similar role
