About the Role

The Pharmacy Refill Specialist plays a critical role in ensuring that patients adhere to their prescribed medication regimens, promoting better health outcomes and enhancing the overall quality of care. As a key member of Harbor Health's multidisciplinary team, you will work closely with pharmacists, clinicians, and other stakeholders to monitor and improve patient adherence. Your expertise will be instrumental in developing and refining workflows, leveraging technology to streamline data collection and analysis, and fostering strong relationships with patients and caregivers.

What You Will Do

  • Conduct interviews with patients and their caregivers to gather information about newly prescribed medications, adherence to treatment plans, and any reported side effects or barriers to adherence
  • Triage medication information and relay it to the appropriate clinical staff, ensuring timely and effective support for patients
  • Collaborate with the Pharmacy Director and IT staff to develop and optimize workflows, leveraging technology to enhance data gathering and analysis
  • Develop and track metrics to demonstrate patient outcomes and adherence rates, providing insights that inform quality improvement initiatives
  • Participate in special projects and perform other duties as assigned, demonstrating your ability to adapt and contribute to the team's objectives
  • Communicate positively and effectively with patients, caregivers, and healthcare professionals, fostering strong relationships and promoting a patient-centered approach to care
  • Analyze data and identify trends, using your analytical skills to inform decisions and drive improvements in patient adherence and health outcomes
  • Stay up-to-date with the latest developments in pharmacy practice, attending webinars, workshops, and conferences to enhance your knowledge and skills
  • Contribute to the development of patient education materials and resources, helping to empower patients and promote better health outcomes

What We Are Looking For

  • Licensed Certified Pharmacy Technician, credentialed and in good standing with the Texas State Board of Pharmacy
  • Minimum 5 years of retail or hospital experience as a Certified Pharmacy Technician
  • High School Diploma or equivalent
  • Strong analytical and problem-solving skills, with the ability to interpret complex data and inform decision-making
  • Excellent verbal and written communication skills, with the ability to communicate effectively with patients, caregivers, and healthcare professionals
  • Proficiency in Microsoft Word, Excel, PowerPoint, and EMR platforms
  • Ability to work independently, with strong judgment and decision-making skills
  • Experience with patient financial assistance programs and patient advocacy
  • Bilingual in English/Spanish (preferred)
