About the Role

The Principal Care Management Care Coordinator plays a vital role in MTC Care's mission to provide exceptional telehealth services to facilities across the nation. As a remote member of the care team, this professional will be responsible for coordinating patient care, tracking patient data, and communicating effectively with medical staff, patients, and families. The role requires a unique blend of technical skills, attention to detail, and excellent communication abilities.

Day-to-day, the Care Coordinator will work closely with clinicians to gather medical information, report concerns, and ensure that all interventions are completed monthly. This will involve utilizing electronic health record systems, Microsoft Excel, and other healthcare technologies to manage patient data and communicate with the medical team. The ability to work independently in a remote setting, with minimal supervision, is crucial for success in this position.

What You Will Do

  • Assist clinicians with gathering medical information for patients between visits, utilizing different electronic health record systems.
  • Report any medical concerns monthly, including falls, medication issues, behavioral concerns, and recent hospitalizations.
  • Complete Principal Care Management (PCM) as assigned, ensuring all interventions are completed each month for approximately 15 patients per day.
  • Communicate effectively with facility staff, colleagues, and medical team members to ensure seamless care coordination.
  • Aid with additional employee trainings as necessary and provide support to newer employees through "shadowing".
  • Report any issues directly to the ancillary program manager and/or Chief Clinical Officer when applicable.
  • Utilize Microsoft Excel to track and analyze patient data, identifying trends and areas for improvement.
  • Develop and maintain a thorough understanding of healthcare technologies, including electronic health record systems and internet/cloud-based applications.
  • Collaborate with the medical team to develop and implement patient care plans, ensuring that all interventions are patient-centered and effective.
