About the Role
The Referral Coordinator position at Midi Health is a critical component of the company's expanding health system and provider partnerships. As a Referral Coordinator, you will be responsible for managing inbound referrals from partner organizations, engaging with referred patients, and coordinating appointment scheduling. What You Will Do
- Manage inbound referrals from health system and provider partners, ensuring accurate intake and documentation
- Engage with referred patients and assist with scheduling their appointments
- Coordinate with the Commercial team to support a seamless patient experience
- Use Zendesk, phone outreach, and referral management systems to interact with patients and manage referrals
- Ensure that referrals are processed efficiently and that patients receive the care they need in a timely manner
- Collaborate with the team to identify areas for improvement and implement process enhancements
- Develop and maintain a thorough understanding of Midi's intake workflows and referral management systems
- Provide exceptional patient care and support to referred patients
- Meet or exceed performance metrics, including referral processing time and patient satisfaction
