About the Role
As a Registered Nurse with Conduit Health Partners, you will be part of a team that provides telephonic triage and care advice to patients. What You Will Do
- Provide telephonic triage to patients, assessing their needs and determining the most appropriate level of care
- Offer care advice and education to patients using decision-making tools and clinical judgment
- Participate in care coordination, partnering with patients to reduce readmissions and enhance chronic disease management
- Schedule provider appointments and facilitate communication between patients and healthcare providers
- Ensure accurate and timely documentation in electronic health records
- Participate in virtual monitoring and subsequent reporting and escalation to support services
- Provide additional support to Conduit Health Partners' business functions as needed
- Participate in process improvement, professional development, and peer review
What We Are Looking For
- Active Compact RN licensure
- Minimum of 1 year of acute care nursing experience
- Triage experience preferred
- Strong clinical skills and judgment
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong organizational and time management skills
- Proficiency in electronic health records, preferably EPIC
- High-speed internet connection with a minimum download speed of 100mbps and upload speed of 20mbps
- Dedicated home workspace with secure physical areas and private access
Nice to Have
- BSN degree
- Experience with telehealth services
- Certification in a relevant specialty, such as telehealth or case management
- Experience with care coordination and patient education
