Registered Nurse (RN) - Triage Specialist - Conduit Health Partners

What You Will Do

• Provide telephonic triage to patients, assessing their needs and determining the most appropriate level of care
• Offer care advice and education to patients using decision-making tools and clinical judgment
• Participate in care coordination, partnering with patients to reduce readmissions and enhance chronic disease management
• Schedule provider appointments and facilitate communication between patients and healthcare providers
• Ensure accurate and timely documentation in electronic health records
• Participate in virtual monitoring and subsequent reporting and escalation to support services
• Provide additional support to Conduit Health Partners' business functions as needed
• Participate in process improvement, professional development, and peer review

What We Are Looking For

• Active Compact RN licensure
• Minimum of 1 year of acute care nursing experience
• Triage experience preferred
• Strong clinical skills and judgment
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong organizational and time management skills
• Proficiency in electronic health records, preferably EPIC
• High-speed internet connection with a minimum download speed of 100mbps and upload speed of 20mbps
• Dedicated home workspace with secure physical areas and private access

Nice to Have

• BSN degree
• Experience with telehealth services
• Certification in a relevant specialty, such as telehealth or case management
• Experience with care coordination and patient education
