About the Role

The Registered Nurse position at Allied Services Integrated Health System is a crucial part of the hospice care team, responsible for overseeing the primary caseload of patients and providing skilled professional nursing care. This role involves working closely with patients, their families, and caregivers to develop and implement comprehensive care plans. The ideal candidate will have a strong understanding of hospice philosophy and principles of death and dying, as well as excellent communication and interpersonal skills.

As part of the interdisciplinary team, the RN will collaborate with other healthcare professionals to ensure that patient needs are met and that care is delivered in a compassionate and responsive manner. This role requires a high degree of autonomy, as well as the ability to work effectively in a team environment. The RN will also provide supervision to Hospice Aides/Homemakers, ensuring that patients receive high-quality care in their homes.

What You Will Do

  • Conduct initial and comprehensive nursing assessments of patient needs
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team
  • Provide skilled nursing care to patients, including pain and symptom management
  • Supervise Hospice Aides/Homemakers and provide guidance on patient care
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Communicate effectively with patients, families, and caregivers to understand their needs and concerns
  • Provide education and support to patients and families on self-care and disease management
  • Participate in ongoing evaluation and revision of care plans to ensure that patient needs are met
  • Maintain accurate and timely documentation of patient care and progress
  • Participate in quality improvement initiatives to enhance patient care and outcomes

What We Are Looking For

  • Graduate of an accredited Nursing Program
  • Current license as a Registered Nurse issued by the Pennsylvania State Board of Nurse Examiners
  • Minimum of one year of general nursing experience, preferably in hospice, home health, medical, surgical, or critical care
  • Certified Hospice and Palliative Nurse (CHPN) certification preferred
  • Ability to work independently and as part of an interdisciplinary team
  • Strong communication and interpersonal skills
  • Ability to manage stress and maintain a professional demeanor in challenging situations
  • Valid PA driver's license and available transportation during working hours
  • Ability to maintain confidentiality of patient information

  • Experience with electronic medical records (EMRs)
  • Familiarity with pain and symptom management interventions
  • Knowledge of hospice regulations and standards
  • Experience working in a home-based care setting
