Job Description

About the Role

The Registered Nurse will oversee a primary caseload of Hospice patients, providing skilled professional nursing care to them, their families, and caregivers.

Responsibilities

  • Provides initial and comprehensive nursing assessments of patient needs
  • Collaborates to develop the Hospice Interdisciplinary Team (IDT) plan of care
  • Implements and reevaluates the plan of care to provide responsive goals and interventions
  • Works closely with other IDT members to ensure all patient and family care needs are met
  • Provides supervision to Hospice Aides/Homemakers

Requirements

  • Graduate of an accredited Nursing Program required
  • Bachelor's Degree in Nursing preferred
  • Certified Hospice and Palliative Nurse (CHPN) certification preferred
  • Minimum of one year of general nursing experience preferred

Nice to Have

  • Experience with pain and symptom management interventions
  • Understanding of Hospice philosophy and principles of death/dying
