Job Description
About the Role
The Registered Nurse will oversee a primary caseload of Hospice patients, providing skilled professional nursing care to them, their families, and caregivers.Responsibilities
- Provides initial and comprehensive nursing assessments of patient needs
- Collaborates to develop the Hospice Interdisciplinary Team (IDT) plan of care
- Implements and reevaluates the plan of care to provide responsive goals and interventions
- Works closely with other IDT members to ensure all patient and family care needs are met
- Provides supervision to Hospice Aides/Homemakers
Requirements
- Graduate of an accredited Nursing Program required
- Bachelor's Degree in Nursing preferred
- Certified Hospice and Palliative Nurse (CHPN) certification preferred
- Minimum of one year of general nursing experience preferred
Nice to Have
- Experience with pain and symptom management interventions
- Understanding of Hospice philosophy and principles of death/dying
