Job Description

About the Role

The RN Intake Coordinator is responsible for screening and evaluating new enrollees, coordinating care initiatives, and supporting the Case Management Program.

Responsibilities

  • Screens potential new enrollees and evaluates their needs
  • Assists Case Team Staff with outreach activities
  • Works with leadership to create support materials
  • Interviews patients and families to determine care needs
  • Provides formal handoff to the appropriate care team

Requirements

  • At least one year of RN work experience
  • Previous RN Case Manager experience preferred
