Job Description

About the Role

As a Senior Integrated Clinic Pharmacist at CenterWell Senior Primary Care, you will play a vital role in delivering high-quality patient care and optimizing medication therapy. Your expertise will be crucial in improving patient outcomes, particularly for those with chronic conditions such as diabetes, hypertension, and heart failure. You will be part of a multidisciplinary team, collaborating with physicians, nurses, and other healthcare professionals to provide comprehensive care.

The role requires strong critical thinking skills, excellent patient counseling abilities, and the capacity to communicate complex medical information effectively. You will report to the Associate Director of Integrated Clinic Pharmacy and will be responsible for staying up-to-date with the latest clinical guidelines and pharmacotherapy advancements.

In this position, you will have the opportunity to work remotely, utilizing your skills to make a positive impact on patient care and outcomes. Your work will be supported by a robust electronic medical record system, and you will be expected to maintain accurate documentation of all clinical interventions and patient care activities.

What You Will Do

  • Evaluate the suitability of pharmacotherapy for high-risk patients with chronic conditions, using evidence-based medicine and clinical judgment.
  • Execute clinical protocols as allowed per organization and state regulatory agencies.
  • Optimize drug coverage benefits for appropriate care and reduced costs, including preferred formulary products, lower cost alternatives, and home delivery pharmacy services.
  • Collaborate with physicians, nurses, and other healthcare professionals to optimize patient care and participate in high-risk rounds with a multidisciplinary team.
  • Provide excellent patient counseling skills and communicate complex medical information to patients and healthcare providers.
  • Engage in disease state education for patients on the appropriate use of medications and self-management.
  • Participate in quality improvement initiatives aimed at enhancing medication safety and efficacy.
  • Conduct post-discharge medication reconciliation for high-risk transition of care patients.
  • Document all activity within clinic EMR and other required pharmacy or clinical management systems.
  • Support CMS Stars and HEDIS quality measures for successful clinic performance.

What We Are Looking For

  • Active and unrestricted Missouri pharmacist license.
  • Willingness to become licensed in additional states for market support.
  • Pharm.D. or other advanced pharmacy degree.
  • 5+ years of experience as a practicing pharmacist.
  • Experience working in a team environment and supporting common goals.
  • Ability to use clinical judgment to make informed decisions about patient care.
  • Excellent patient counseling skills and the ability to communicate complex medical information.
  • Familiarity with electronic medical record (EMR) systems.
  • Ability to identify and implement solutions for improved efficiencies and outcomes.

Nice to Have

  • Clinical residency in ambulatory care or similar discipline.
  • 2+ years working in an ambulatory care/primary practice setting.
  • Board certification in geriatric pharmacy or pharmacotherapy.
  • Multilingual (preferably English/Spanish).
