What You Will Do
- Collaborate with healthcare staff to develop, implement, and evaluate individualized care plans for patients
- Coordinate comprehensive care services, including patient education, medication management, and transitions of care
- Provide telephonic support and guidance to patients, families, and healthcare providers
- Document patient interactions and care activities in a timely and accurate manner
- Participate in interdisciplinary case conferences and team huddles to ensure coordinated care
- Identify patients who require case management and develop personalized care plans to meet their unique needs
- Collaborate with the oncology care team to develop and implement clinical pathways and care protocols
- Participate in quality improvement activities to identify opportunities for improving patient outcomes
- Develop and maintain relationships with patients, families, and healthcare providers to ensure seamless care coordination
- Utilize Excel and other software applications to track patient data, outcomes, and care activities
- Stay up-to-date with the latest developments in medical technology, treatments, and care protocols
What We Are Looking For
- Current nursing licensure in the United States
- Minimum 3-5 years of experience in nursing, preferably in oncology or a related field
- Strong knowledge of clinical specialties, patient care, and care coordination
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities
- Proficiency in Excel and other software applications
- Strong analytical and problem-solving skills
- Ability to work collaboratively with interdisciplinary teams
