About the Role

The Telehealth Physician role at Wider Circle represents a unique opportunity for medical professionals to make a meaningful impact in the lives of vulnerable populations. By conducting initial virtual visits, assessing medical needs, and establishing clear clinical plans, you will be instrumental in setting the direction for members' care journeys. This role is not about traditional panel-based practice; instead, it focuses on understanding the patient and setting direction, with a dedicated support team handling ongoing engagement. The position is part of a new clinical pilot that integrates physicians, care navigators, and community support teams to create a more coordinated and human approach to care.

The role entails working closely with a care team, including Community Health Workers and Licensed Clinical Social Workers, to ensure a smooth handoff and continuity of care. Your clinical insight will also be crucial in refining program workflows during the pilot phase. What You Will Do

  • Conduct initial telehealth visits with enrolled members to understand their medical needs, barriers to care, and overall health goals.
  • Assess medical needs, care gaps, and barriers to accessing care to establish a comprehensive view of each member's situation.
  • Establish a clear clinical plan and care priorities, ensuring that each member receives appropriate and timely care.
  • Collaborate with care team members to ensure a smooth handoff and continuity of care, leveraging the strengths of each team member to provide holistic care.
  • Provide clinical insight to help refine program workflows during the pilot phase, contributing to the development of innovative care models.
  • Support a patient-centered approach that addresses social and medical needs together, recognizing the interplay between these factors in overall health.
  • Work in a flexible, evolving program environment, adapting to changing needs and circumstances.
  • Participate in regular team meetings and feedback sessions to continuously improve care delivery and team performance.
  • Stay updated on the latest clinical guidelines, research, and technologies to ensure evidence-based practice.

What We Are Looking For

  • MD or DO with an active, unrestricted medical license in Tennessee.
  • Board certified or board eligible in Family Medicine, Internal Medicine, or a related primary care specialty.
  • Comfort working with Medicare, Medicaid, or complex populations, understanding the unique challenges and needs of these groups.
  • Strong communication skills and the ability to build trust quickly in a virtual setting, empathizing with patients and their families.
  • Interest in care coordination, health navigation, or whole-person care models, with a willingness to innovate and adapt.
  • Comfort working in a startup-like, evolving program environment, being flexible and resilient.
  • Telehealth experience preferred, with knowledge of digital health technologies and their applications in care delivery.
  • Ability to work independently as a 1099 independent contractor, managing time and priorities effectively.
