Job Description

About the Role

The Vice President of Population Health will play a critical role in leading and scaling Covera Health's Protect Seniors program, a core pillar of the company's AI-driven quality platform. This role sits at the intersection of Customer Success and Operations, owning the performance and growth of a proven population health model while serving as the primary relationship owner for health plan and PCP partners. The ideal candidate will have a strong background in population health, experience working with healthcare organizations, and a passion for improving patient outcomes.

As a key member of the Executive Leadership Team, the VP of Population Health will drive strategy, execution, and outcomes, operating at the center of Covera's ecosystem across payers, providers, and patients. This role will require strong leadership skills, the ability to build and maintain relationships with key stakeholders, and a deep understanding of the healthcare industry.

The VP of Population Health will work closely with cross-functional teams, including Customer Success, Operations, and Product, to drive growth and improvement in population health outcomes. This will involve partnering with health plans and provider organizations to implement and scale population health programs, as well as working with the product team to develop and refine AI-driven quality insights and tools.

What You Will Do

  • Lead and scale Covera Health's Protect Seniors program, a core pillar of the company's AI-driven quality platform
  • Develop and execute population health strategies to drive growth and improvement in patient outcomes
  • Build and maintain relationships with key stakeholders, including health plan and PCP partners
  • Collaborate with cross-functional teams, including Customer Success, Operations, and Product
  • Drive strategy, execution, and outcomes, operating at the center of Covera's ecosystem
  • Partner with health plans and provider organizations to implement and scale population health programs
  • Work with the product team to develop and refine AI-driven quality insights and tools
  • Analyze data and metrics to inform population health strategies and drive improvement
  • Develop and manage budgets and resource allocation plans
  • Hire, train, and manage a team of population health professionals

What We Are Looking For

  • 8+ years of experience in population health, healthcare management, or a related field
  • Strong background in population health, including experience working with healthcare organizations
  • Experience working with health plans and provider organizations
  • Strong leadership skills, with the ability to build and maintain relationships with key stakeholders
  • Deep understanding of the healthcare industry, including current trends and challenges
  • Experience working with AI-driven quality insights and tools
  • Strong analytical skills, with the ability to analyze data and metrics to inform population health strategies
  • Experience managing budgets and resource allocation plans
  • Strong communication and interpersonal skills

Nice to Have

  • Experience working with electronic health records (EHRs) and health information exchanges (HIEs)
  • Knowledge of healthcare regulations, including HIPAA and Affordable Care Act
  • Experience working with data analytics and visualization tools
  • Certification in population health, healthcare management, or a related field
