About the Role
The Vice President of Population Health will lead and scale Covera's Protect Seniors program, a core pillar of the company's AI-driven quality platform. This role requires a unique blend of customer success, operations, and relationship-building skills. As a key leader, you will partner closely with the Executive Leadership Team to drive strategy, execution, and outcomes, operating at the center of Covera's ecosystem across payers, providers, and radiology partners.Day-to-day, you will own the performance and growth of a proven population health model, serving as the primary relationship owner for health plan and PCP partners. Your expertise in Medicare Advantage will be crucial in driving measurable clinical and financial impact at scale. As a cross-functional leader, you will work across departments to ensure coordinated action and optimal business outcomes.
What You Will Do
- Own end-to-end performance of the Population Health (Protect Seniors) value chain, from data intake through gap identification, partner engagement, gap closure, and outcome validation
- Build and manage relationships with Customer executive sponsors, align on strategic objectives, manage expectations, and ensure overall satisfaction with the Population Health program
- Partner closely with the Growth/Commercial team to align go-to-market strategy with execution capabilities on a market-by-market basis
- Drive adoption of Population Health across the market by engaging ACOs, PCP groups, and radiology facilities to drive adoption and engagement with programs
- Steer the Executive Leadership Team in defining and executing the Population Health strategy
- Serve as the primary point person for primary care provider partners in the market, driving meaningful engagement with clinical findings
- Own implementation and deployment of Population Health programs, driving pilot success and ensuring conversion into long-term, scaled partnerships
- Work deeply with a cross-functional internal team to plan and execute on Population Health priorities
- Establish KPIs for Population Health and track progress against goals
What We Are Looking For
- 8+ years of experience in population health, preferably with a focus on Medicare Advantage
- Proven track record of driving growth and operational excellence in a similar role
- Exceptional relationship-building and customer success skills
- Strong understanding of healthcare operations and clinical quality improvement
- Experience working with payers, providers, and radiology partners
- Ability to work in a fast-paced, dynamic environment and adapt to changing priorities
- Strong analytical and problem-solving skills, with expertise in Adjust and Excel
- Bachelor's degree in a related field, such as healthcare administration or public health
